Claim of Charity Benefits

Requirements:  Medical/death certificate; original OR of hospital bills; medicines bought; certificate of registration or ID

 Location: Campus Clinic

Fee: None

Steps:

  1. Present requirements to the nurse. The nurse will issue blank Charity Benefit Claim form.
  2. Fill up the form and submit to the nurse. The nurse will validate the form and issue claim stub containing the date of release of the benefits. Processing time takes three (3) weeks.
  3. Return on the date of release and claim the benefits at the Cashier’s Office. Claimant must bring school ID or COR and the claim stub.

Claim of Group Accident and Health Protection Program (GAHPP) Benefits

Requirements: School ID or certificate of registration; Medical/death certificate; original official receipt of hospital bills and medicines bought; narrative/police report

 Location: Campus Clinic

Fee: None

Steps:

  1. Present requirements to the nurse. The nurse will issue blank GAHPP form.
  2. Fill up the form and submit to the nurse. The nurse will validate the form and issue claim stub containing the date of release of the benefits. Processing time takes three (3) weeks.
  3. Return on the date of release and claim the benefits at the Cashier’s Office. Claimant must bring school ID or COR and the claim stub.

Replacement of Damage Library Card

Requirements: Damaged library card and one (1) piece 1x1 ID pictures (white background, not computer generated)

Location: Campus Library

Fee: P50.00

Steps:

  1. Surrender the damaged library card and submit the 1x1 ID picture to the library personnel. A blank library card and replacement form will be issued to the client.
  2. Fill up library card and replacement form, and then submit to the library personnel. The documents will be validated, nd the card laminated.
  3. Pay replacement fee of P50.00.
  4. Receive the library card and the provisionary receipt.

Re-issuance of Lost Library Card

Requirements: Certificate of Registration and one (1) piece 1x1 ID pictures (white background, not computer generated)

 Location: Campus Library

Fee: P60.00

Steps:

  1. Pay re-issuance fee of P60.00 to the library personnel. A provisionary receipt and blank Affidavit of Loss form will be issued to the client.
  2. Fill up the form and have it signed by the Director or the Associate Director of Administrative Services.
  3. Submit the signed Affidavit of Loss form with the 1x1 ID picture to the Library personnel. A blank library card will be issued to the client.
  4. Fill up library card, and then submit to the library personnel. The documents will be validated, and the card laminated.
  5. Receive the library card.

Application for Scholarship

Requirements:

  • Three (3) pieces 1x1 ID pictures;

  • DILG Scholar- certification from LGU concerned;

  • DOST Scholar- certification from DOST;

  • Privilege Scholar of BPSU Employee- Parents certificate of employment;

Issuance of Certificate of Good Moral Character to Transferring Student

Requirements: one (1) documentary stamp; honorable dismissal or transfer credential

Location: Guidance Office of the Campus

Fee: None

Steps:

  1. Present requirements to Guidance personnel. The personnel will check the requirements. If found complete, the certificate will be prepared.
  2. Sign logbook and receive the Certificate of Good Moral Character.
