Application for Scholarship

Requirements:

• Three (3) pieces 1x1 ID pictures;

• DILG Scholar- certification from LGU concerned;

• DOST Scholar- certification from DOST;

• Privilege Scholar of BPSU Employee- Parents certificate of employment;

   

• CSC/ Publication Scholar- certification from Office of Student Affairs;

• Academic Scholar- certification of grades;

• Sports and Cultural Scholar- certification from Sports Director/ Cultural Adviser;

• ROTC Scholar- certification from the commandant;

• High school Valedictorian/Salutatorian- certification from the principal;

• AFP/ Veterans Scholar- certification from AFP.

Location: Guidance Office of the Campus

Fee: None

Steps:

1. Present requirements to Guidance personnel. The personnel will check the requirements. If found complete, application form will be issued to the client.
2. Fill up and submit accomplished application form to Guidance personnel. Processing of application usually takes three (3) working days.
3. Secure approved application forms, photocopy approved form and submit original copy to the cashier.
